Every weapon, glove, ring, shield and aura in Overgeared Hero now has a tier and a reason. Celestial Shield is the one S+ piece, burn gear fills S, and 13 pieces are marked not worth investing in.
The top of the list
Celestial Shield is the only S+ piece. Once per fight, dropping to 1% HP makes you invincible for 4 seconds and heals you to full. That is a second life on nearly every build. The one setup that can skip it is a block-stacking spear Rebecca.
S is the burn core. Primal Flame makes basic attacks apply Burn, and every socketed item that burns adds another stack, so it's the first weapon to take to Mythic unless you play Rose. Gloves of Plague give socketed weapons 10% Haste and a 50% chance to add 2 more stacks of the enemy's most common debuff, and two pairs beside Primal Flame are the damage core of the build. For rings, Garnet Ring is better in short fights and Archwizard's Ring in long ones. Most burn builds level one of each.

Spike is the second build
Spiral Spear sits in A+ and that isn't a knock on it. Take it if you own Rose. Everyone else keeps it as the swap for floors where burn stalls. Thunderbolt Spike strikes twice and charges the skill on every basic hit, which suits spell and multi-hit heroes, but multi-hit hurts you against bosses that return damage with Spike.
